Look for proven enterprise experience across major EHR systems (Epic, Oracle Health, athenahealth), deep HL7 and FHIR expertise, healthcare-specific compliance certifications (HIPAA BAA, HITRUST, ISO 27001), and a senior engineering team that has shipped at your scale. Ask any prospective EHR integration company for named case studies and technical references — not just a logo wall.

How much does an EHR integration project cost?

Accepted Answer

It depends on scope — but here's how scope actually works in this category, so you can sanity-check whatever number a vendor quotes you.
Three variables drive cost more than anything else:

Compliance scope. A read-only integration with one EHR under straightforward HIPAA scope is a different project than a bi-directional integration under FDA Software as a Medical Device (SaMD) scope, 42 CFR Part 2 substance-use restrictions, or CLIA laboratory requirements. Compliance overhead can easily double or triple engineering time on the same technical scope.
Number of EHR systems. A single-vendor integration (just Epic, just athenahealth) is meaningfully cheaper than a multi-EHR project. Each additional EHR adds its own sandbox credentialing, OAuth scope, conformance testing, and vendor-specific implementation quirks.

Direction and depth of data flow. Reading data is cheaper than writing data. Writing data is cheaper than bi-directional sync. Bi-directional sync with conflict resolution and clinical-workflow integration is the most expensive pattern.
Rough engagement-size ranges we see in practice:

Focused single-EHR read-only integration: 2–4 month engagement, smaller team
Bi-directional multi-EHR integration, standard HIPAA scope: 6–12 month engagement, dedicated team

How long does an EHR integration project take?

Most enterprise EHR integration projects take 3–9 months from kickoff to production. The variance is large because four factors compound — and the one that delays most projects has nothing to do with engineering.The four phases, with realistic time ranges:

Discovery and conformance audit: 1–3 weeks. Reviewing your existing data architecture, the EHR systems involved, the interoperability standards (HL7 v2, FHIR R4, USCDI v3 or v5), and the compliance framework that applies to your project.

Architecture and design: 2–4 weeks. Defining the integration pattern, API contracts, error-handling, and the test strategy. Faster if the project follows a pattern we've shipped before; longer if it's novel.

Implementation, testing, and integration QA: 2–6 months. The actual build. Length depends on bi-directional vs. read-only, number of EHR systems, and whether real clinical data is in scope (which triggers de-identification or BAA-protected test environments).

Parallel-run and go-live: 2–6 weeks. Running the new integration alongside the existing system, validating data fidelity, and cutting over without disrupting clinical workflows.

The biggest source of timeline slip is usually sandbox access — not engineering.Getting credentialed into an Epic, athenahealth, or Oracle Health sandbox with the right OAuth scopes routinely takes 4–8 weeks and is outside the EHR integration services company's direct control. For Epic specifically, App Orchard / Vendor Services onboarding can run 4–12 weeks depending on your use case and whether you already have a customer sponsor. 
We start the sandbox-access process on day one of the engagement, in parallel with discovery, specifically to absorb this delay.

The major commercial EHRs, plus custom and proprietary systems. But the more useful answer is how deeply we've worked with each — because every EHR integration project is shaped more by the specific vendor's quirks than by the standards on paper.

The EHRs we integrate with most often:

Epic. Kanda is an Epic Services Vendor. We work with Epic's App Orchard / Vendor Services program, including App Market and Connection Hub integrations, FHIR R4 and DSTU2 APIs, MyChart integrations, Bridges interfaces, and Interconnect engine work. Our Epic experience includes both customer-facing app development and direct hospital-side integration projects.

Oracle Health (Cerner). Code Console / CernerCare API work, Millennium-based integrations, HealtheIntent platform integrations, and SMART on FHIR app development. We've worked with the Cerner platform under its Cerner branding and through the Oracle Health transition.

athenahealth. athenaOne integrations via the Marketplace partner program, athena's FHIR API and proprietary REST endpoints, and bi-directional clinical data integrations. We are familiar with athena's specific authentication and rate-limiting patterns.

Allscripts / Veradigm. Sunrise, TouchWorks, and Professional EHR integration work. HL7 v2 interface engine integration via Mirth Connect, Rhapsody, or Cloverleaf.

NextGen. NextGen Enterprise and Office integrations, NextGen Connect (formerly Mirth Connect) interface engine work.

MEDITECH. Expanse and Magic-era integrations, including MEDITECH's FHIR APIs and traditional HL7 v2 interfaces.

eClinicalWorks. eCW Cloud and ambulatory integrations, including their REST API and HL7 interfaces.

Yes. We sign HIPAA Business Associate Agreements with each client and are ISO 27001:2022 certified, SOC 2 Type II compliant, and operate under HITRUST-aligned controls. We work in FDA-regulated environments (21 CFR Part 11), CLIA laboratory contexts, and under 42 CFR Part 2 substance-use-disorder restrictions.
